Semaglutide and other GLP-1 medications have brought more attention to this topic because they help many people lose a lot of weight. As people lose weight, their faces change too.

Dr. Yalif says the name can make the issue sound more uniform than it is. “Two patients can lose the same amount of weight and need completely different things,” he says. “One may have hollow temples. One may have loose skin through the jawline. One may love the way their face looks.”

The term has become popular, but the right treatment always depends on each person's unique face.

Facial Fat Does Not Leave in a Neat Pattern

The face holds fat in separate pockets and layers. The cheeks, temples, under-eye area, and folds around the mouth all rely on that volume for support.

Weight loss can reduce volume in these areas in uneven ways. One part of your face might look more sculpted, while another looks more tired. Cheeks can flatten, folds around the mouth may stand out more, and the jawline might look sharper, but with some loose skin.

This is why simply adding filler is not always the best solution.

Some wait until their weight has leveled out before considering facial fat grafting or surgery. "I do not want to treat something that is still changing," Dr. Yalif says. "A good plan considers the patient's health goals and allows time for the face to adjust."

Filler Has a Place. So Does Facial Fat Grafting.

Filler is the treatment most people are familiar with. It can restore support through the cheeks, temples, jawline, and folds around the mouth with little downtime. In the right hands, a modest amount can make a face look refreshed rather than filled.

Dr. Yalif treats filler as a tool for structure, not decoration. “A small amount in the right place can change the balance of the face,” he says. “The patient should look rested. They should still look like themselves.”

Filler can work well for someone with localized volume loss who wants a non-surgical option. It can also give a patient a sense of how restored volume may change their face before they consider a longer-term procedure.

Facial fat grafting plays a different role. The procedure uses a patient’s own fat, taken from another area of the body, and placed into selected areas of the face. It can work well for people who have lost volume in several areas, including the cheeks, temples, and under-eye region.

“Fat grafting gives us a way to restore volume with the patient’s own tissue,” Dr. Yalif says. “For the right patient, it can bring back a softness that weight loss has taken away.”

Fat grafting deserves an honest conversation. Part of the transferred fat absorbs during healing. Future weight changes can affect the face again. Dr. Yalif reviews those details before recommending the procedure.

Skin Laxity Calls for a Different Conversation

Filler can add support. Facial fat grafting can replace lost volume. Neither treatment can pull up significant loose skin.

This is when facelift surgery may become an option.

Weight loss can leave the lower face with jowls, skin laxity, and less definition along the jawline and neck. A facelift repositions deeper facial tissue and addresses excess skin. A neck lift can refine the area below the jaw. Some patients benefit from a facelift with fat grafting, which brings together lifting and volume restoration.

“A lift and fat grafting do different jobs,” Dr. Yalif says. “A lift addresses tissue descent and loose skin. Fat grafting restores support and softness. Some faces need one approach. Some need both.”

The best treatment is based on a careful exam, not on what is currently popular.

No one needs to treat every facial change that comes with weight loss. A leaner face can look great. A little more definition through the cheeks or jaw may be exactly what someone wanted.
