Breast Implant Filling
Dr. Yalif offers implants by Mentor, one of the most trusted names in breast implant safety and technology. These implants can be filled with either medical-grade saline solution, cohesive silicone gel, or highly cohesive silicone gel, sometimes called “gummy bears.” Choice of implant filling is one of personal preference. Most patients believe the cohesive gel implants feel and look more like natural breasts and are used in more than 80% of breast augmentation procedures performed in the United States. Silicone implants arrive at our office pre-filled, so Dr. Yalif makes a slightly larger incision than he uses for saline implants, which he fills after they’re inserted.
Breast Implant Size
Most patients discuss their goals in terms of bra sizes—you may want to increase from a B to a D, for example. But implants are measured in cubic centimeters (cc) because bra sizes can vary a great deal depending on the brand. By listening to patients describe the types of results they want and performing a thorough physical exam, Dr. Yalif can help recommend implant sizes that can meet patients’ goals. Breast augmentation is one of the most popular cosmetic procedures in the U.S. and Dr. Yalif uses the most advanced techniques when he performs this surgery. He creates a surgical plan based on your consultation which includes a specific incision site and how he will place the implants.
Incision Site
Dr. Yalif inserts implants through one of 3 types of incisions:
Inframammary: This is at the base of the breast, just above the crease where it connects to the chest wall. This is the most common location because they offer excellent access to the breast pocket and are well-concealed beneath the breasts.
Periareolar: These incisions are made along the lower edge of the areola, the darker skin surrounding the nipple. This is a very discreet incision, but you should inform Dr. Yalif if you plan to breastfeed in the future because there is a small risk of disrupting the milk ducts.
Transaxillary: A surgeon makes an incision in the underarm area. There is no scar on the breast, but the incision can be seen when lifting your arms while wearing a bikini or tank top. It’s not suitable for patients who get silicone gel implants.
Breast Implant Placement
Dr. Yalif uses the Keller Funnel® to insert breast implants for optimal sterility and safety. The funnel enables him to place the implants without touching them, lowering the risk of complications. He places the implants either in front of or behind the chest muscle.
A subglandular placement involves inserting implants behind the existing breast tissue but in front of the muscle. This is possible for patients with enough existing tissue to adequately camouflage the implant.
A submuscular placement involves inserting implants behind the pectoral muscle. This is a popular choice because the chest muscle provides more coverage for the implant, and the result often looks and feels more natural.
Dr. Yalif also offers a subfascial implant placement. Using this technique, he creates a pocket behind a thin strip of connective tissue called the fascia—but in front of the muscle itself. It’s an appropriate approach in some cases and can be discussed with Dr. Yalif during the consultation.
Can Breast Augmentation Be Combined With Other Procedures?
Many women who come to our practice have had children and are looking to recapture their pre-pregnancy bodies. Pregnancy, childbirth, and breastfeeding can take a physical toll on the breasts and abdominal area. A mommy makeover combines cosmetic breast surgery, such as a breast lift combined with augmentation, along with body contouring procedures that typically include a tummy tuck and liposuction. Dr. Yalif’s experience makes him an excellent choice if you’re considering a mommy makeover.
Do You Offer Fat Transfer Breast Augmentation?
Yes, we do. Breast enhancement using fat from another area of your body is a technique we offer for women who want a modest increase in the size of their breasts without having implants inserted. Fat transfer is a technique that’s used in multiple ways, including as an alternative to synthetic fillers and for buttock augmentation.
How Long Does It Take to Recover from Breast Augmentation?
Recovery from breast augmentation surgery is generally well tolerated. You can anticipate a week or so of pain from the incisions; expect muscle pain as well if the implants were placed behind the muscle. The second week is usually considerably better with much less sharp pain with some muscle soreness and aching. In addition, the implants will start to feel a bit softer as the tissues accommodate the implant. Dr. Yalif may allow you to resume exercise at this point while wearing a sports bra or may ask you to wait a few more weeks if the surgery was more extensive. The implants will continue to soften and settle over the next 4 to 8 weeks.
Breast Augmentation FAQs
What happens to your breast implants if you have children after a breast augmentation?
Having children causes changes to your breasts. The size of the ducts increases as they prepare to produce milk and the surrounding soft tissue of the breast also stretches. The implant remains the same, but the tissue around it can change dramatically. Similarly, when you stop breastfeeding the gland may shrink as it stops producing milk. This results in the breast deflating and appearing to fall off of the implant.
Can you breastfeed after a breast augmentation?
Yes, you can! We recommend choosing the inframammary incision to avoid risk to your duct tissue. Even if we use an incision along the lower edge of the areola, though, it should not result in enough scar tissue to interfere with breastfeeding. It’s important to remember, however, that not all women can breastfeed infants whether or not they get breast implants.
How painful is breast augmentation surgery?
The discomfort depends on the techniques used during the procedure. The area where the incisions are made will feel similar to recovering from a deep cut but can be managed with pain medication. Implant placement also affects your experience. If the implant is placed above the muscle, there is some ache but it is generally mild. Placing the implant below the muscle causes more of a deep ache for several weeks. While we do prescribe pain medication, few patients need to take all of them. Most patients tolerate the recovery quite well, whether we go above or below the muscle.
Is breast augmentation safe?
Yes, it’s safe! Risks associated with any surgery, such as anesthesia, are part of breast augmentation, of course. Complications also occur, including capsular contracture (when scar tissue develops around the implant capsule). This occurs in fewer than 5% of the cases. Breast asymmetry is uncommon but it can happen. Even less common risks are infection or bleeding around the implant. Implants may rupture or leak, but again this rarely occurs. Extremely rare is a risk of Breast Implant-Associated Anaplastic Large Cell Lymphoma (BIA-ALCL), a type of cancer that develops in the capsule your body forms around an implant. It is highly treatable when caught early and is associated with certain types of implants. We discuss this issue thoroughly during your consultation. Finally, breast implants are not designed to last forever and we recommended exchanging or removing implants after about 10-15 years. This means there is a 100% chance of reoperation in most patients who elect to get breast augmentation.
